Did you know that many visible signs of ageing can be traced back to volume loss? As the facial fat atrophies, we start to develop folds around the nose and mouth, and cheeks look a little hollow. This lost volume can be replaced by dermal fillers which results in filling of grooves, smoothening of wrinkles, augmenting the lips, and restoring a more youthful appearance. Dermal filler is widely chosen therapy across the world for facial rejuvenation and reversing the signs of ageing, as it is a quick, safe and effective way to look younger.
Dermal fillers are composed of gel-like material that are injected below the
skin to restore volume loss, smooth out lines and wrinkles, and enhance
As we age, the facial bones undergo resorption, subcutaneous fat decreases, and the ligaments loosen. As a result, there occurs deflation and sagging of the face. We develop hollowness and deep folds. The shape of the face changes with the lower part of the face becoming more bulky.
Dermal fillers help in restoring the lost volume and restructuring the lost shape of our face.
Dermal fillers are an excellent tool to reverse the signs of ageing and
enhance once appearance.
Below mentioned are some of the effects that dermal fillers have:
Augmentation & enhancement of thin lips
Contouring & restoring volume to sunken cheeks, temples, forehead
Decreasing & softening of facial creases and wrinkles around nose and mouth (smile lines and marionette lines)
Smoothening out a chin crease
Improving facial symmetry
Reconstructing contour deformities in the face
Removing the shadow below the eyes
Dermal fillers can be very helpful in those with early signs of ageing, or as a value-added part of facial rejuvenation surgery.
There are several different types of dermal fillers. The most commonly used
is hyaluronic acid, which is available in different formulations, chosen
according to the indication.
Hyaluronic Acid is naturally found in your body. It is present in high concentrations in soft connective tissues and in the fluid surrounding the eyes. It is also found in joint fluids, and skin.
Hyaluronic acid is the most popular types of dermal fillers used worldwide.
Hyaluronic acid can be used for the treatment of hollowness and grooves of face, facial contouring, and even to fill depressed scars.
Following are the common indications of hyaluronic acid injections:
Marionette lines at the corners of the mouth
Smoker's lines; vertical lines around the mouth
Jaw line re-definition
Brand names include: Juvederm, Hylaform,, Perlane, Restylane
Hyaluronic acid fillers differ in chemical makeup, longevity and have varying degrees of softness. Softer fillers are used in the lips, for example, while sturdier fillers might be desired to enhance cheek bones. Your dermatologist will determine with you the best type and volume of filler needed for your particular areas of concern.
You may opt for dermal fillers if you:
Are physically healthy
Have a positive outlook and realistic goals in mind for the improvement of your appearance
Are committed to maintaining good skin health
A dermal fillers procedure includes the following steps:
Step 1 – Facial assessment & mapping.
The doctor will evaluate your facial appearance and examine the areas of
your face to be augmented.
Strategic points on your face will be marked as the appropriate injection sites for the filler. Photographs will be taken of the areas to be treated.
Step 2 – Cleansing & anaesthetising.
The injection sites will be cleansed with alcohol swabs.
Pain at the injection site may be ameliorated by use of an anaesthetic ointment to numb the skin. The fillers are pre-mixed with lignocaine (anaesthetic agent), so the injections are usually easily tolerated.
Step 3 – The injection
Hyaluronic acid gel is carefully injected at the pre-determined sites. The
improvement is visible instantly. One can instantly see the lifting effect,
contouring and re-positioning of the facial tissues, and filling of grooves and
lines. Any lumps or bumps are smoothened out by gently massaging the
area. Results are assessed, and additional filler is injected where ever
Depending on the number of areas to be treated, the whole process may be as short as 15 minutes, or as long as an hour.
Step 4 – Clean up & recovery. Once satisfactory results are obtained, the face is again cleaned and antibacterial cream applied.
You can resume most activities right away, but it is generally recommended
that you avoid intense physical activity for the first 24-48 hours to minimise
swelling and bruising.
There might be slight redness and swelling at the injection points, which subsides in couple of days.
It is advisable to drink lots of fluids (3-4 litres per day) for 1 week post filler treatment that will help in hydrating the product and enhancing the results.
The results of soft tissue augmentation using dermal fillers are apparent immediately and will last for 12-24 months.
Dermal fillers is a single sitting procedure.
Depending on the indication being treated and the product used, the effects
of dermal fillers can last from 12-24 months.
The results can be enhanced by maintaining a healthy skin with appropriate products and skin treatments.
No. Once the hyaluronic acid gets dissolved in the tissues, you resume whatever condition you had in the starting of the treatment. There is no worsening.
Dermal filler injections is an absolutely safe and effective treatment if done by
Minor temporary adverse effects like swelling, bruising, pain, injection point erythema may occur rarely.
Severe complications from dermal fillers are uncommon.
Potential risks include:
Acne-like skin eruptions
Bruising, bleeding from the injection site, swelling
Damage to the skin resulting in scarring
Infection at the injection site
Superficial placement of filler leading to its palpability under the surface of the skin
Skin rash with itching
Under- or over-correction of wrinkles
Skin necrosis (ulceration or loss of skin from disruption of blood flow)
Your own body’s fat can be harvested, processed, and injected as dermal fillers to bring out similar effects as those by hyaluronic acid.
Fat injection is a more elaborate procedure than hyaluronic acid fillers.
First step is to identify a donor area from which fat can be harvested. This is
usually the abdomen or buttocks. Using a liposuction cannula, fat is removed
from the donor area, it is processed in a machine, and then injected in the
The grafted fat then has to redevelop a blood supply in order to survive. About 50% of the fat injected will survive, and the surviving fat will last forever.
The results of fat injection are less predictable. Usually, more than one
sittings are required to get the desired results.
Fat injections are associated with more discomfort, both in the recipient as well as in the donor area. This is a minor surgical procedure that can be performed in the surgeon's treatment room or in an operating room.
